How flat is the freq response of the A12?

I bought mine from eBay. It starts to decrease output at 5K.

Assuming your measurements are correct, there's definitely something wrong with it. What caps are installed at C1, C6, and C20-C22?

Referred to +4dBu @ 1KHz and 40dB of gain, frequency response is -0.3dB at 20Hz and 20KHz; -3dB at <5Hz and >65KHz. Installing a 1500pF at C6 by mistake would put response down about 3dB at 5KHz. You may also have a bad op-amp.
