U4 transistor got too hot after SC25 was installed

the U4 transistor started to smell after about 60sec at power up, once I installed the SC25 op-amp.

All tests and adjustments were to spec before the Sc25 was installed.

Had C4 Bass Ackwards. Works good now

i did exactly the same thing with c5 yesterday. there was a little bit of a smell, but no smoke. c5 changed to a darker orange color but went back after it cooled down. could this have damaged u5 or c5? in better news, it sounds incredible.
