PS03 D5 Diode Blowing

Diode D5 on my PS03 1.4 blows periodically. What could cause this?

This is still an issue. This D5 diode appears to connect to what is presumably the multi tap transformer that supplies the various voltages. I put in a bigger diode; 3A, 400V but it still blew after some time. Any clues?
