A12 pulsing light and no sound

I have built 4 of these and for some reason this one doesn't work. When I put all 4 in the chasis the power light starts to blink. I narrowed it down to one of my A12's. If I take it out the problem stops. I tried switching out the op amps with the same problem happening in the same one. I went back through all of the parts and can't seem to find anything in backwards. It was working and then after I turned it off for a couple of days and came back is when the problems started. I tried putting them all in different positions to see if the power supply was weird.

So you still see the symptom with the op-amp removed?

Yes.

You almost certainly have a component installed incorrectly, probably one of the tantalum bypass capacitors next to the op-amp.
