U4 very hot after SC25 is placed

I read the similar previous post. All caps are properly oriented.
Everything measures as it should until the last step when I put SC25 into place. U4 gets very hot almost immediately. nothing happens to adjacent caps. Measurement between pins 4 and 6 of op amp show 9.46v regardless of changes done to R16. have inspected work and reflowed all solder joints with no changes.
