Hi.

When nothing is plugged into the front and I touch the switches, I get a buzz. It is most noticeable in the center position.

I've checked and rechecked my work but can't find the problem.

Any ideas?

Does the module work as expected with the inputs terminated?

Yes. As far as I can tell.
