Distorted sound on one of my T15's

I'm getting +16 and -9V DC at the output. The diodes are all correct, and I've checked all of the other components as well. These voltages are present at D5-D6 and D7-D8 respectively. Bad U3??? Any ideas let me know! Thanks!
